§ Pillar 01 · Healthcare · Family Bridge

You can’t fly back every Tuesday.

Family Bridge is the service we are building within Ìwẹrẹ Care to connect diaspora children with parents at home: one health centre contact, one volunteer physician, one monthly digest. It is not open yet. Ìwẹrẹ Care is in pilot at the Jakpa and Ugbege health centres in Warri North, with The Roli Mabo Foundation.

§ 01 · Included

Care you’d give if you lived next door.

What we are building. Not an app. Not a platform. People at a real health centre, and a volunteer physician.
§ 01

One health centre contact

The plan: each enrolled family is linked to one member of staff at their health centre, starting with the pilot centres at Jakpa and Ugbege in Warri North. One person. One WhatsApp thread. One relationship.

§ 02

Monthly vitals and chronic review

Blood pressure, pulse, oxygen level, glucose and weight, measured at the health centre once a month and tracked over time. A volunteer physician reviews the trend before it reaches you.

§ 03

Medication review

We intend to check every prescription against your parent’s current medication list, and to work out reliable refills with the health centre.

§ 04

Monthly digest email

One email a month. Vitals, medications, visits, any concerns. If there’s nothing urgent, it takes a minute to read. If something needs attention, it says so at the top.

§ 05

A way to reach us when it matters

We are working out how families will raise urgent concerns and how the health centre will reach you. We will publish the details, honestly, before enrollment opens.

§ 06

Shared dashboard

The Ìwẹrẹ Care caregiver portal we are building. The idea: you see what the health centre sees, and your siblings can join too. One family, one view.

§ 02 · Families we bridge

The stories are still to come.

We will share families’ words here, with their permission, once the service is running.

§ 03 · From Day 0

Enrollment to steady state.

How we expect it to work once enrollment opens.
Step 1

Interest

You tell us about your parent and where they live. Our team will contact you.

Step 2

First conversation

Someone from the health centre speaks with your parent and arranges a first visit.

Step 3

Baseline vitals and care plan

Your parent visits the health centre. A volunteer physician reviews the results and helps write a care plan.

Step 4

First digest email

You receive the first monthly digest. Trends become useful after a few months.

Step 5

Chronic care

If your parent lives with high blood pressure or diabetes, a volunteer physician reviews their trend each month.

Ongoing

Steady state

You stop worrying every day. You worry when the health centre says to worry. That’s the trade.

§ 04 · Questions caregivers ask

The honest answers.

Not listed? Ask us →
01

Is Family Bridge open now?

Not yet. Ìwẹrẹ Care is in pilot, run by INC-USA with The Roli Mabo Foundation at the Jakpa Health Centre (2 beds) and the Ugbege Health Centre (5 beds) in Warri North, Delta State. Family Bridge is the service we are building on top of that pilot. Tell us you are interested and our team will contact you.

02

Does Family Bridge replace a parent’s actual doctor?

No. If your parent has a trusted local doctor, the aim is to work alongside them, sharing records with your parent’s consent so nothing falls between desks.

03

What about siblings who also want access?

We plan to let several family members share the same dashboard. Useful for siblings spread across the US, the UK and Lagos.

04

How much will it cost?

We are working on a sliding scale so that families can contribute what they are able to. We will publish pricing before enrollment opens.

05

What if the parent lives far from a health centre?

Today the pilot centres are at Jakpa and Ugbege in Warri North. Sites at Warri Central and Sapele are planned, not yet open. Tell us where your parent lives and we will tell you honestly what we can and cannot do.

06

What happens in an emergency?

In an emergency, go to the nearest hospital. Family Bridge is not an emergency service. We will share how the health centres handle urgent referrals once those arrangements are confirmed.
